Your Personal Information
At 740 Dental we respect your right to privacy and are committed to protecting any personal information we hold about you. We keep records about you, your treatment and care so we can provide you with safe and effective dental services.
How We Look After and Safeguard Information About You
The practice owner is the Data Controller for the personal information we collect and hold. We aim to ensure your information is securely stored, treated confidentially and only accessible to authorised staff who need it for your treatment, care or support.
Information Recorded
The information that we hold may include (but is not limited to):
• Your basic personal details; for example your name, address, date of birth, contact details and next of kin.
• Clinical and dental records concerning your diagnosis, treatment, medical history (including social and dental), medication and any communication relevant to your dental care.
• Information about treatment provided to you, including records from other health professionals or relatives who support your care.
Sharing Information
We will not share your personal information with anyone outside the practice unless one of the following circumstances applies:
• You ask us to do so.
• You give us specific permission to share your information.
• We are required by law to share it, for example for safeguarding, public protection or legal compliance.
• We believe in good faith that disclosure is required to protect someone’s life, or in the public interest where the law allows.
When we share information, we will ensure the recipient is under a legal duty to keep it confidential and secure.
Your Right to View Your Records
You have the right to access your dental and clinical records held by 740 Dental. If you would like a copy of your records, please submit your request in writing to the Practice Manager. We will provide access free of charge for standard requests. If you require a copy we reserve the right to charge a reasonable fee for administrative costs in line with our Data Protection Policy. We aim to respond within 40 working days of receiving your request.
Confidentiality
We are committed to maintaining your confidentiality. All our staff contracts include an obligation to protect patient information. We follow professional codes of conduct and apply appropriate security measures to safeguard your information from unauthorised access or disclosure.
Legal Basis for Processing
The legal basis for processing your personal and clinical data is for the purposes of diagnosis and the provision of dental care or treatment. Where required, we also rely on your informed consent.
Policies and Procedures
We have a number of key policies and procedures in place to manage the quality of care and information security at 740 Dental. These include our Confidentiality Policy, Data Protection Policy, Complaints Procedure and Privacy Notice. A paper copy of any of these is available on request from our Practice Manager.
Complaints
We welcome your feedback, suggestions or complaints as part of our commitment to continuous improvement. If you have any concerns about the service we provide, please raise these with the Practice Manager, or the Practice Owner. We will treat you with respect, investigate your concerns promptly and aim to respond in the shortest possible time.
